Important information about double action carabiners

From E-Connection Projects BV, we received an ‘incident / near miss report’ about using a double action carabiner on your lanyards when working in a wind turbine. Please click here to read the report and prevent an incident.
In short: when using the double action carabiner (shown in the picture) in a wind turbine, you may run the risk of opening the hook when descending through the hatch. The side of the hatch may open the first mechanism, lifting the carabiner and the back ring subsequently may open the second mechanism. By using one of the other types of hooks, you prevent a hazardous situation.
